Continuing & Complex Care

Care Outcomes dedicated staff receive specialised training to meet the needs of clients’ continuing and complex care as those requirements change and develop.

The specialist services we offer include full medication management, the safe use of hoists, assessment of equipment needs, manual moving and handling manoeuvres, assisting with mobility, falls prevention practice and the correct use of continence equipment.

Our staff are training to identify activities that enable clients to maintain independence at home. This includes clients who have Alzheimer’s disease, vascular dementia, Lewy body dementia and Pick’s disease. In depth knowledge, teamed with a comprehensive understanding of the signs and symptoms of dementia, including the effect it has both on the client and carers, allows the increasing complexity of the condition to be managed at home for the maximum time possible.

Many conditions change and develop, symptoms become more complicated and the associated care needs must be continually monitored and adapted when necessary. That’s why we provide our staff with enhanced training. A care plan is only successful when it addresses the current needs. Requirements change over time but the level of our care and our continuing commitment to maintaining our clients’ dignity and independence remains the same.
